Mission

To elevate the health of our community by increasing access for all to quality health services, especially the uninsured and underinsured.

 

Vital Statistics

Community Health Center of Fort Dodge, Inc. (CHCFD) was originally established in 1999 as the Fort Dodge Free Clinics. The CHCFD will transition the free clinics into Federally Qualified Health Centers in April 2006 with funding received from the Bureau of Primary Health Care. The organization will provide services on a sliding fee schedule to the uninsured and undersinsured adults and children of the community.
